Functional reconstruction of lower extremity nerve injuries

Peripheral nerve injuries (PNI) in the lower extremity are an uncommon but highly morbid condition. Recent advances our understanding of physiology and microsurgical techniques have inspired renewed faith surgery sparked a creative renaissance tools, approaches, reconstructive schemas available to surgeons management PNIs. In this article, we review literature provide principles-based approach for surgical PNIs with emphasis on functional reconstruction after complete injury. General principles include early diagnosis electrodiagnostics imaging, exploration, opting tendon transfers when primary injured is unfavorable (e.g., delayed reconstruction, unavailability proximal or distal stumps, long regenerative distance). The goal should be restore independent gait, so roles major neuromuscular units during gait cycle informs selection donor nerves tendons transfer.
